How Do I Start Services

If you want help for a mental health or substance use problem, you have found the right place.  Follow these quick and easy steps to get the help you need.

I have Insurance or Can Private Pay

Call 972-422-5939 – Request to Speak to an Insurance Verification Specialist

LifePath Systems is a contracted provider with multiple insurance carriers, Market Place insurance, Medicaid Managed Care plans, traditional Medicaid, Medicare, and Children’s Health Insurance Plans.  Call our insurance verification specialist to ensure we accept your plan, verify your co-pay/deductible, and schedule with a provider.

I Do Not Have Insurance or the Ability to Private Pay

Step OneGather the following documents to bring with you to the Open Access clinic

  • Photo ID
  • Social Security Card
  • Two (2) Proofs of Residence in Collin County
  • Proof of Total Household Income or Verification of Assistance Form (for you and your spouse)
  • Proof of the Number of Family Members in Your Household
  • Proof of Legal Custody or Guardianship (if client is 3-17 years of age or an incapacitated adult)
  • Insurance Card(s)

Step Two –Walk-in to one of our Open Access clinic locations, Monday through Friday, 8 am to 3 pm.

Step ThreeBehavioral Health Screening

Meet with a qualified mental health professional to discuss your symptoms to determine clinical eligibility for services. Community referrals are provided for those who do not qualify for services.

 Step FourFinancial Eligibility Assessment

Complete a financial eligibility assessment based on a sliding fee scale to determine your ability or inability to pay for services.

 Step Five Mental Health Intake Assessment and/or Substance Use Assessment

Meet with a licensed clinician to complete an assessment, determine the type and level of care needed, and establish a treatment plan.

Step SixChoose Your Provider

LifePath contracts with a number of behavioral health providers and is also a provider of services.  Our clinicians will assist you in scheduling an appointment with your chosen provider.  We want to make sure you leave with an appointment for all of the services you need.
