SUMMARY LifePath Systems, the Local Authority for persons with Intellectual and Developmental Disabilities in Collin County, has an opening for an eligibility caseworker.  The position will work with individuals, families and organizations seeking information about services, eligibility and resources. Work is performed under the general supervision of the Program Administrator with broad latitude for the use of initiative and independent judgment.


  • Serve as the point of contact for individuals and families.
  • Scheduling and performing intake interviews and basic assessments; managing various waiting lists; and serving as a resource for employees, individuals, families, school districts and other social service organizations regarding available supports and resources. This includes governmental benefits such as Medicaid, Medicare, TANIF, etc.
  • Perform face-to-face interviews and assessments in order to gather information necessary to establish eligibility for services.
  • Performs and supports various Continuity of Services with state facilities, Medicaid Waiver programs and community ICFMR’s
  • Support eligibility determination (DMR) by the Center Psychologist to gather necessary data to DMR or Endorsement of DMR reports
  • Must comply with and meet Case management Operating Instructions and requirements of the State Medicaid Plan.


  • Knowledge of the human services delivery system.
  • Ability to communicate effectively in person on the telephone and in writing;
  • Able to organize and prioritize a variety of assignments while managing time and productivity;
  • Form and maintain working relationships with consumers, stakeholders and other employees;
  • Perform basic assessments, personal interviews and act in a professional manner at all times;
  • Have basic computer knowledge, word processing abilities, and be able to learn new software.
  • HCS/TxHmL Medicaid Waiver experience preferred.


  • Bachelor’s degree (B. A.) in a social services related field is required.
  • Two to four years related experience and/or training; or equivalent combination of education and experience.
  • Bi-lingual (English/Spanish) is a plus


Must have a valid Drivers License.
